Comments:

A monitoring inspection was conducted on June 7, 2023 from approximately 10:40 am to 12:45 pm. There were 9 children present, ranging in ages from 12 months to 5 years with two caregivers supervising for a total of 19 points. A total of five child records were reviewed and the records for the two caregivers and one adult household member were reviewed. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. During the inspection, the children were observed during outside play and rest time. The caregiver's interactions with the children were positive. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	Based on record review and interview, the provider failed to ensure a sworn disclosure statement and criminal history record check was completed for one household member within 30 days of the person turning 18 years old. Evidence: 1. Household member #1 became 18 years of age on 4/6/2023 and did not have documentation of a sworn disclosure statement or criminal history record check. 2. During an interview with the provider, household member #1's birthdate was confirmed as correct and the provider acknowledged the sworn disclosure statement and criminal history record check has not been completed.

Description:	Based on record review and interview, the provider failed to annually review contact information for 3 children to ensure the information was correct. Evidence: 1 Child #1(start date 8/30/21), Child #4 (start date 6/22/20) and child #5 (start date 7/2/19) did not document annual reviews for 2022 to ensure emergency contact information was correct. 2. During an interview with the provider, it was acknowledged that the annual reviews were not completed.

Description:	Based on record review and interview, the provider did not ensure to obtain a current Report of Tuberculosis Screening published by the Virginia Department of Health or a form consistent with it documenting the absence of a communicable form for adult household member prior to coming into contact with children. Evidence: 1. The record for adult household member #1 did not contain documentation of a tuberculosis screening. 2. During an interview with the provider, it was acknowledged that a tuberculosis screening has not been conducted.

Description:	Based on documentation reviewed and interview, the Provider failed to ensure all household pets (dogs or cats) are vaccinated for rabies and treated for fleas, ticks or worms as needed. EVIDENCE: 1. The provider's 3 dogs were observed in the backyard. 2. Vaccinations for the 3 dogs expired on 2/15/2023. 3. During an interview, the provider confirmed the vaccinations were expired.
