Comments:
An announced monitoring inspection was conducted April 25 2023 between 11:00am and 12:00pm. 4 children were observed in the direct care of 2 caregivers. A walkthrough of the home was conducted. A review of children's records, caregiver records, pet records, emergency preparedness plans, first aid kits, and the outdoor play space was conducted. Description: Based on review, the provider did not obtain documentation of additional immunizations every six months for a child 2 years of age and younger.

Evidence: Child 2 record did not contain documentation of an updated immunization record. The document on file is dated September 11 2021. The child began care September 20, 2021. Child 2 has a birth date of June 23, 2021.

Plan of Correction: Not available online. Contact Inspector for more information.
