Comments:
A renewal inspection was conducted Thursday, June 8, 2023. The licensing inspector was onsite conducting the inspection from approximately 10:15am to 12:30pm. There six non-resident children, ranging in ages from 19 months to four years, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and two caregivers? records.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations was documented on the violation notice issued to the program.

Standard #: 8VAC20-800-800-B
Description: Based on documentation review, the provider did not document in writing review the emergency plan at least annually and update the plan as needed.

Evidence:
The emergency preparedness plan was dated as being reviewed on 04/01/2022.

Plan of Correction: I will review the plan to see if any changes need to be made and document the review in writing.

Standard #: 8VAC20-800-60-B
Description: Based on review of five children?s records, one child?s record did not contain immunization records and results of the health examination.

Evidence:
1. The record for Child #5 (first day of attendance 12/5/2022) did not contain immunization record and results of the health examination.
2. The provider stated she had a copy of the records on the computer but was unable to locate the records during the inspection.

Plan of Correction: I have scanned the records before but could not located them during the inspection. I will continue to look for them to print and add to the record or request another copy.

Standard #: 8VAC20-800-230-A
Description: Based on review of two caregivers? records and interview, the provider did not maintain written documentation of each caregiver's orientation.
Evidence:
1. The record for Caregiver #2 did not contain documentation of orientation.
2. The provider stated orientation was completed but it was not documented.

Plan of Correction: I did not realize there was an orientation form to complete. I will print the form and document the orientation that was completed.
