Technical Assistance:
The provider must send documentation to the Licensing Inspector that the background checks have been requested no later than 10 days following this notification dated 06/05/2023 on the Acknowledgement Form. Once the background check is received by the provider, the results must be forwarded to the licensing inspector.

Comments:
An unannounced monitoring inspection was conducted on June 5, 2023. There were 4 children present with 2 caregivers supervising. The home, children's records, caregiver records, household member records, cribs, cots, changing pads, diaper creams, evacuation drills, and first aid kits were reviewed. The backyard was also inspected. Areas on non-compliance were cited on violation notice. Standard #: 22.1-289.036-B-2
Description: Based on a review of caregiver records and household member records, the provider failed to ensure that all caregivers and adult family members submit to fingerprinting and provider personal descriptive information described in subdivision B 2 of ? 19.2-392.02;

Evidence:
Caregiver #1's criminal history record check expired on 01/30/2023.
Household Member #1's criminal history record check expired on02/12/2023.

Plan of Correction: The provider will ensure that criminal history reports are obtained.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, the provider failed to obtain the names of persons other than the custodial parents who are authorized to pick up the child.

Evidence:
Child #4's record does not contain documentation of persons other than parents who are authorized to pick up the child.

Plan of Correction: Provider will request information from parents.
