Comments:
An unannounced monitoring inspection was conducted on 06/05/2023 from 11:50am to 1:25pm. There were 6 children ages 2 years old to age 12 years old (12 points) with the Provider. The programming area, attendance, outdoor playground area, menus, Provider, Assistant and Household member records, 6 children?s records, drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in lunch, watching television and outside play. Diapering and handwashing procedures were also observed. The Provider was current with certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 6/14/23. Please email me at angela.dudek@doe.virginia.gov with any questions.

Violations:
Standard #: 22.1-289.035-A
Description: Based on review of 2 Caregiver records, the provider did not obtain a sworn disclosure every five years from the date of the last sworn disclosure.

Evidence: The record for Caregiver #2 did not contain documentation that a sworn disclosure had been completed every five years from the date of the last sworn disclosure. The last sworn disclosure was completed 3/30/18.

Plan of Correction: Signed a new sworn disclosure.

Standard #: 22.1-289.036-A
Description: Based on review of the Provider record, the provider did not complete a sworn disclosure every five years from the date of the last sworn disclosure.

Evidence: The record for the Provider did not contain documentation that a sworn disclosure had been completed every five years from the date of the last sworn disclosure. The last sworn disclosure was completed 4/5/18.

Plan of Correction: Signed a new sworn disclosure.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider did not ensure that potentially poisonous substances were stored in an area inaccessible to children.

Evidence:
1)A bottle of air freshener was stored on a shelf in the bathroom where it was accessible to children.
2)Face cleaner was stored in an unlocked cabinet that was accessible to children.

Plan of Correction: Moved the air freshener higher out of reach.

Moved face cleaner out of the bathroom cabinet.

Standard #: 8VAC20-800-500-D
Description: Based on observation, the Provider did not ensure that outdoor play equipment has ?S? hooks that are tightly closed and that openings above the ground that are closed on all sides should be smaller than 3.5 inches or larger than 9 inches to prevent head entrapment.

Evidence:
1)On the swing set, the ?S? hooks on all swings were open and not tightly closed as required
2)On an area next to the slide there is a gap above the ground measuring 5 inches by 29 inches and a gap measuring 6.75 inches by 29 inches that poses a risk of head entrapment.
3)Under a platform next to the slide there is a gap above the ground measuring 3.6 inches by 26 inches that poses a risk of head entrapment.
4)Under the slide there is a gap above the ground measuring 6.25 inches by 26 inches that poses a risk of head entrapment.
5)At the suspension bridge, there is a gap above the ground measuring 4.5 inches by 26.5 inches and a gap measuring 6 inches by 26.75 inches that poses a risk of head entrapment.

Plan of Correction: Ordered an "S hook tool" to close "S hooks" farther.
Put boards in mentioned spaces.
