Comments:
A monitoring inspection was conducted on June 8, 2023 from approximately 11:15 am to approximately 12:55 pm. There were four children in care, ranging in age from 2 to 9 years and six resident children present with one caregiver supervising for a value of 13 points.

The inspector reviewed compliance in the areas of administration, care of children, physical plant, physical equipment and environment, staffing and supervision, programming, special care and emergencies and nutrition.

A total of two child records, the provider's record and two household member records were reviewed. The children were observed having lunch, playing on the computer, and napping.The provider home is equipped with toys and supplies and items were available to the children. The provider's interactions with the children were positive.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Violations:
Standard #: 8VAC20-800-830-C
Description: Based on record review and interview, the provider did not ensure emergency evacuation drills were documented.

Evidence:
1.The most recent evacuation drill documented during the inspection was for 2/1/2023.
2. During an interview, the provider acknowledged monthly drills were completed but not documented.

Plan of Correction: Will start documenting drills monthly starting today

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not ensure that a child was adequately immunized before the child attended the family day home.

Evidence:
The first day of attendance for Child #1 was 5/2/2023. The immunization record was dated 5/4/2022.

Plan of Correction: Will make sure I have them before they start

Standard #: 8VAC20-800-680-B
Description: Based on observation and interview, the provider failed to ensure children's hands are washed with liquid soap and warm running water.

Evidence:
1.The four children in care were observed being directed to wash their hands after having lunch.
2.The bathroom designated for the children in care was observed as not having any liquid soap.
3. During an interview, the provider acknowledged their was no liquid soap available for the children.

Plan of Correction: Will purchase a case of liquid soap today
