Comments:
A monitoring inspection was initiated on 6/15/2023 and concluded on 6/15/2023. There were no children present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The inspection started at 10:05 am and concluded at 11:00 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-940-B
Description: Based on observation and interviews the provider failed to ensure that an operable thermometer is available to monitor refrigerator and freezer compartment temperatures.
Evidence:
The provider stated that she does not have an operable thermometer available to monitor the refrigerator and freezer.

Plan of Correction: The provider will ensure that the refrigerator and freezer has an operable thermometer.

Standard #: 8VAC20-800-240-A
Description: Based on observation the provider failed to ensure that all Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence:
The light switch in the bathroom located in the day care area does not have a cover exposing the wires behind the switches. This bathroom is used by the children in care.

Plan of Correction: The provider will have the switch plate cover replaced and monitor that all outlets and switch boxes have covers.

Standard #: 8VAC20-800-260-E
Description: Based on observation the provider failed to ensure that Bathtubs, buckets, and other containers of liquid accessible to children shall be emptied immediately after use.
Evidence:
1. Located in the kitchen was a mop bucket full of dirty water.
2. Located on top of the trash can in the kitchen was a container of water with rags soaking.

Plan of Correction: The provider emptied the buckets and containers.
