A monitoring inspection was initiated on 06/13/2023 and concluded on 06/13/2023. There were 9 children present, ranging in ages from 1 year to 4 years (22 pts), with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 provider/staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-800-690-D Description: Based on observation, the provider failed to ensure that the diapering surface was nonabsorbent and washable.
Evidence:
1. The diapering changing pad on the changing table had several small visible rips on the pad and were nonabsorbent and washable.Plan of Correction: I am planning on changing it out tomorrow.
