The following standards were discussed:
8VAC20-800-430. Stairs.
A. Children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs.
B. Accordion expansion gates and pressure mounted gates shall not be used as protective barriers at stair openings.
C. Children over the age of two shall not have access to stairs with three or more risers that do not have protective barriers or guardrails on each side.
D. Protective barriers or guardrails on sides of stairs shall be constructed to prevent a child from climbing over, crawling or falling through, or becoming entrapped.

Comments:
An unannounced renewal inspection was conducted today from 11:20am-12:45pm. There were 5 children and 2 resident children (18 points) directly supervised by 2 staff. The home, 3 staff records, 4 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely and playing in the backyard. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review, the family day home did not obtain a completed sworn statement or affirmation form from a staff member prior to employment.

Evidence:
Staff #3's (DOH: 1/20/2023) record does not contain a completed sworn statement or affirmation form.

Standard #: 8VAC20-800-700-A
Description: Based on review of medication and provider interview, it was determined that a child had a medication on site without a written medication authorization form.

Evidence:
Child #3 has a prescription medications on site without a medication authorization form completed and signed by the parent.

Standard #: 8VAC20-800-740
Description: Based on review of documentation and interview, the provider did not keep a record of medication administered to children.

Evidence:
A prescription medication was administered to Child #3 on 6/13/2023 at 12:05pm. The written documentation did not include the name of the child to whom medication was administered, the amount and type of medication administered to the child, the name of the caregiver administering the medication, any adverse reactions, and any medication administration error.
