Comments:

A monitoring inspection was conducted on June 22, 2023 with three caregivers and the provider. There were 9 children present, ranging in age from9 months to 5 years, totaling 24 points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 caregivers were reviewed. The children were eating lunch and laying down for rest time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-800-50-A
Description:	Based on observation and staff interview, the written record of children in attendance for June 22, 2023 was not accurate. Evidence: Child 5 was present, but was not on the written attendance record.
Plan of Correction:	Child 5 was added to the attendance record.
Standard #:	8VAC20-800-60-A
Description:	Based on provider interview, the provider failed to maintain a record at the family day home for each enrolled child. Evidence: the provider could not locate the file for Child 4.
Plan of Correction:	The provider will locate the child's file.
Standard #:	8VAC20-800-70-A
Description:	Based on review of four children's files, the provider failed to have documentation that each parent had been provided in writing required information. There was no documentation that Child 1, Child 2, and Child 3 had received the VDOE model form titled "Information for Parents" or an equivalent. There was no documentation that that Child 2 had received the VDOE model forms titled "Provisions of the Emergency Preparedness and Response Plan", "Policy on Administration of Medication", and "Liability Insurance Declaration" or an equivalent.
Plan of Correction:	The provider will get the forms signed by each parent for the file.
Standard #:	8VAC20-800-170-A
Description:	Based on review of three caregiver files, the provider failed to obtain a tuberculosis test or screening for each caregiver at the time of hire. Evidence: Caregiver 3 (date of employment 5/24/2023) did not have documentation of a tuberculosis test or screening.
Plan of Correction:	Caregiver 3 will get documentation of a tuberculosis test or screening.
