Standard #: 8VAC20-800-60-B Description: Based on children's records, and confirmed by Provider, children's records did not contain name, address, and telephone number of each custodial parent's place of employment.
Evidence: There is no work phone number for each custodial parent of Child #3.Plan of Correction: Provider will obtain the work phone numbers for the parent of Child #3.
