An unannounced monitoring inspection was conducted todaybetween the hours of 1:15pm and 2:20pm. There were 11 children directly supervised by the provider and one additional caregiver. The home, 2 caregiver records, 3 children?s records, 1 household member?s record, evacuation drills, and emergency supplies were inspected. Children were observed during naptime. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Evidence:
1. The record for Child #1 (start date 1/2/2023) did not contain documentation of a physical.Plan of Correction: I will call the parent tonight to request the physical.
