Comments:
A monitoring inspection was conducted on 06/20/2023 from 11:46 a.m. ? 2:24 p.m. There were 8 children present during the inspection and one provider present. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five (5) children?s records, 1 provider record, and 1 household member record were reviewed. The information gathered during the inspection determined violations with applicable standards. A
violation notice was issued. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 07/3/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on observation, the provider did not ensure that the first aid kit contained at a minimum, supplies required by 8VAC20-800-760-A-1.
Evidence:
The first aid failed to contain triangular bandages.

Description: Based on observation and interview, the provider failed to ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the
home.
Evidence:
On 06/20/2023, the inspector observed the provider caring for 8 children. The
maximum licensed capacity of the home is 7 children. Provider reported trying to be helpful to parents by providing care despite knowing she was exceeding her licensed capacity. Provider reports she has submitted a request to county zoning to increase the home?s capacity.

Description: Based on observation, the provider failed to ensure that cots used for children shall have linens consisting of a top cover and bottom cover or a one-piece covering that is

Description: Based on observation, the provider failed to allow only school-age children to play outdoors while the caregiver is indoors if the caregiver can hear the children playing outdoors.
Evidence:
Upon arrival for inspection, the provider and children were observed outside in the back yard with eight (8) children. The provider was observed to enter the home multiple times for several minutes at a time leaving children to play outside. Four (4) of the children were not school age.
