Comments:
A monitoring inspection was conducted on June 28, 2023 from approximately 9:50 AM to 11:40 AM. There were 9 children present in the center, ranging in ages from 20 months to four years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures

Description: Based on interview, record review and observation, it was determined the provider did not ensure compliance with the home's policies that have been disclosed to the parents.

Evidence:
1. The record for provider #1 and provider #2 did not have documentation of a MAT or AMAT certification.
2. The Medication Administration - Decision to Administer provided to families states that the Provider will not administer any medications - prescription and / or non prescription.
3. The provider stated child #1 has a food allergy and there is prescription medication at the home in the event the child has an allergic reaction.

Standard #: 8VAC20-800-220-A
Description: Based on review of caregiver records, the provider failed to ensure that whenever the family day home has a agreed to administer prescription medications or nonprescription medications, the administration shall be performed by a caregiver who has satisfactorily completed a training program for this purpose developed or approved by the Board of Nursing and taught by a registered nurse, licensed practical nurse, doctor of medicine or osteopathic medicine or pharmacist.

Evidence:
1. The record for provider #1 and provider #2 did not have documentation of Medication Administration Training. The provider accepted emergency medication from the parents of Child #1.
