Comments:
A Subsidy Health and Safety Inspection was conducted on Wednesday, June 28, 2023, to review supplemental health and safety requirements. The inspector was on site from 9:50am to approximately 12:05pm. During the inspection, there were a total of ten children in care, totaling 18 points, in the direct care of one caregiver. Upon arrival, the children were observed watching television in a designated space on the first floor of the family day home. Later, the children were observed transitioning to their designated space downstairs. The children were observed taking bathroom breaks, during free-play, and playing outside. All areas used for childcare were inspected. The family day home has age and stage appropriate materials and supplies. The caregiver was observed having positive interactions with the children in care. The first aid kit and non-emergency supplies were inspected and found complete. Per the provider, there are no pets in the home, no firearms, transportation is not provided, and medication is not administered at this time. Two caregivers' records and five children's records were reviewed.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-250-B
Description: Based on a review of two caregivers' records and interview, the vendor did not ensure one caregiver completed the Virginia Preservice Training for Child Care Staff sponsored by the Department of Education within the first 90 days of employment.

Evidence: 1) The record of Caregiver #2, employed on 06/01/2020, did not contain documentation that the caregiver completed the Virginia Preservice Training. 2) During interview, the provider confirmed Caregiver #2 has not completed the training.

Plan of Correction: Caregiver #2 will complete the required training.

Standard #: 8VAC20-790-250-C
Description: Based on a review of two caregivers' records and interview, the vendor did not ensure orientation training for one caregiver was completed prior to the caregiver working alone with children and within seven days of the date of employment or the date of subsidy vendor approval.

Evidence: 1) The record of Caregiver #2, employed on 06/01/2020, did not contain documentation that the caregiver completed orientation training on the following required topics within seven days of the date of subsidy vendor approval, which was effective 10/08/2020: 1. Playground safety procedures; 2. Responsibilities for reporting suspected child abuse or neglect; 3. Confidentiality; 4. Supervision of children, including arrival and dismissal procedures; 5. Procedures for action in the case of lost or missing children, ill or injured children, medical and general emergencies; 6. Medication administration procedures, if applicable; 7. Emergency preparedness plan as required in 8VAC20-790-420 B; 8. Procedures for response to natural and man-made disasters; 9. Prevention of shaken baby syndrome or abusive head trauma including coping with crying babies and fussy or distraught children; 10. Prevention of sudden infant death syndrome and use of safe sleeping practices; 11. Caregivers who work with children who have food allergies shall receive training in preventing exposure to foods to which the child is allergic, preventing cross contamination and recognizing and responding to any allergic reactions; and 12. Transportation.

2) During interview, the provider confirmed Caregiver #2 did not complete the required orientation training.

Plan of Correction: Caregiver #2 will complete the required training.

Standard #: 8VAC20-790-250-F
Description: Based on a review of two caregivers' records and interview, the vendor did not ensure caregivers who work directly with children completed the department's health and safety update course annually.

Evidence: 1) The record of Caregiver #2, employed on 06/01/2020, did not contain documentation that the caregiver completed the department's health and safety update course. 2) During interview, the provider confirmed Caregiver #2 has not completed the health and safety update course.

Plan of Correction: Caregiver #2 will complete the required training as soon as possible and will complete it annually moving forward.
