Technical Assistance:
Discussed training hours will be reviewed at the next inspection. Discussed the VA Central Registry for the two new household members.

Comments:
An unannounced renewal inspection was conducted today from 10:36am-11:30am with the provider on site. There were 2 resident children present during the inspection. A sample of 1 child?s file and 4 caregivers/household member's files were reviewed. The children were engaged in the following: toy play and watching tv. The physical space, attendance records, first aid kit, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Violations:
Standard #: 8VAC20-800-830-A
Description: Based on review of the emergency response drill record, the provider did not practice the emergency evacuation procedures monthly with all caregivers and children in care during
all shifts that children are in care.
Evidence:
1. The provider did not practice the emergency evacuation drill in February 2023, March 2023, April 2023 and June 2023.

Plan of Correction: I will do it every month.

Standard #: 8VAC20-800-200-A
Description: Based on interview, the provide did not orient the assistant by the end of their first week of assuming job responsibilities.
Evidence:
1. The provider stated the assistant started employment in March 2022, but did not have record of orienting the assistant.

Plan of Correction: I will do the orientation with him.

Standard #: 8VAC20-800-240-A
Description: Based on observation, areas and furnishing of the family day home inside and outside were not maintained in a safe and operable condition.
Evidence:
1. In the playroom, the paint had chipped where a nail had popped out of the wall.
2. The playground ladder to the slide was bent.
3. The children's play sink was missing the sink and faucet handle.

Plan of Correction: I will take the ladder off. I will find the 2 pieces or throw it away. I will repair the paint.

Standard #: 8VAC20-800-320-B
Description: Based on observation, the provider failed to install child resistant protective covers on all unused electrical outlets accessible to children under five years of age.
Evidence:
1. In the playroom, 3 unused outlets were not covered with protective covers.
2. In the family room, 1 outlet was not covered with a protective cover.
3. In the playroom near the front door, one outlet near the window was not covered with a protective cover.
4. In the dining room, 3 outlets were not covered with protective covers.

Plan of Correction: I will cover them today.

Standard #: 8VAC20-800-500-C
Description: Based on observation, the provider's stationary outdoor playground equipment did not meet all requirements.
Evidence:
1. The providers playground equipment was not firmly anchored with ground supports and was under a hard surface of dried dirt.

Plan of Correction: I will take the ladder off and I will fix the playground equipment.
