The Licensing Inspector initiated and completed an unannounced monitoring inspection on 6/23/2023. The inspector was on site from 12:20pm until 12:45pm.
Inspector was on site to review providers records for renewal application.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.036-A Description: Based on record review, the provider failed to obtain an updated search of the criminal history check after 5 years.
Evidence: During the inspection on 6/23/2023, the provider failed to provide an updated Criminal History check for provider #1. The last Criminal History check for provider #1 was 5/24/2018.Plan of Correction: Fingerprints were received on 6/29/2023.
