Comments:
An unannounced monitoring inspection was conducted today from 10:35am-11:25am with the provider on site. There were 3 children with 1 caregiver. A sample of 3 children's files and 2 caregivers/household member's files were reviewed. The children were engaged in the following: toy play. Interactions between the children and caregiver were positive. The site was clean and organized. The physical space, attendance records, first aid kit, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Standard #: 8VAC20-800-770
Description: Based on observation, the provider did not have a working battery operated weather band radio and extra batteries.
Evidence:
1. The provider had a wind up weather band radio, but did not have one working with batteries.

Plan of Correction: I will add the batteries today.

Standard #: 8VAC20-800-880
Description: Based on review of the menu, the provider did not follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence:
1. Monday Menu: Baked Chicken, Broccoli, Bread and milk.
2. Tuesday Menu: Bean and cheese burrito, carrot, tortilla and milk.
3. Wednesday Menu: Meatloaf, mashed potato, break and milk.
4. Thursday Menu: Meatballs, spaghetti, tomato sauce and milk,
5. Friday Menu: Fish, bread, pears and milk.
6. It was missing one vegetable or fruit component each day.

Plan of Correction: I will add the components today.

Standard #: 8VAC20-800-60-B
Description: Based on review of children's records, two children's records did not contain all required information.
Evidence:
1. Child #1's record (start date 1/02/2023) was missing both work phone numbers and the medical insurance company and policy number.
2. Child #3's record (start date 7/03/2023) was missing the immunization record.

Plan of Correction: I will ask the parents for it today.

Standard #: 8VAC20-800-90-A
Description: Based on review of child records, the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the child attended.
Evidence:
1. Child #3's record (start date 7/03/2023) was missing the immunization record.

Plan of Correction: I will ask the parents for it today.

Standard #: 8VAC20-800-280
Description: Based on observation, the potentially poisonous substances were not stored in areas inaccessible to children.
Evidence:
1. A container of Lysol, Clorox cleaner and comet bleach were under the sink in the kitchen cabinet and was not locked and within reach of children.
2. A container of Armoral, 409 cleaner, nair spray and Febreze were under a bathroom cabinet that the children use. The cabinet was not locked and within reach of children. There was a bottle of Lysol behind the toilet.

Plan of Correction: I will move the cleaning supplies and put a lock on the cabinets.
