Comments:
An unannounced non-mandated monitoring inspection was conducted on July 6, 2023. The provider was available during the inspection. There were 11 children present, ranging in ages from 3 months to 2 years, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision. No children records were reviewed and 1 caregiver record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on observation and interview on July 6, 2023, the provided notice of the Superintendent's intent to revoke the license of a child day program was not posted in a prominent place at each public entrance of the facility to advise consumers of serious or persistent violations.
Evidence: 1. The notice of intent was not observed to be posted at each public entrance of the facility.
2. When asked where the notice of intent was located the provider directed the inspector to a clipboard hanging on the wall, which contained two separate folders. The notice of intent was located in a protective sleeve within the second folder.

Plan of Correction: The Notice of Intent was posted by the public entrance but not in a way VDOE deemed acceptable. Provider fixed this at the visit and both inspectors said the correction "rectified" the violation. Since the inspection, provider has spoken to licensing supervisors and has moved the NOI and compliance plan to another wall to better display them separately per document as stated from a higher up in the office. I am taking her suggestion on all posting of said documents from here on out as she is in charge of my consent agreement plan.
