Comments:

An unannounced renewal inspection was conducted on 7/6/2023. There were 9 children, ages 14 months ? 9 years, totaling 20 points and 3 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records, 2 caregiver records and 1 household member record were reviewed. The children were observed during center time and lunch time. The inspector arrived for the inspection at 10:20am and departed at 12:06pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Description:	Based on the review of two caregiver records, the provider failed to ensure that all background checks were complete. Evidence: Caregiver 1 most recent criminal background was dated 2/27/2018 and on the day of the inspection did not have the required 5 year repeat check documented.
Plan of Correction:	Provider obtained updated criminal background check.

Description:	Based on the review of two caregiver records, the provider failed to ensure for assistants and substitute providers documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: Caregiver 2 start date was 5/15/2023 and on the day of the inspection did not have two or more references documented prior to employment.
Plan of Correction:	Provider will obtain two or more references for assistant.
