Technical Assistance:
Discussion on required documents for staff and children records.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am-11:00am. There were 4 children supervised directly by 1 staff. The home, 3 staff records, 2 children?s records, 2, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed waking up from rest time. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.036-B-4
Description: Based on review of three staff records, the center did not have an original central registry report within 30 days of employment of a staff member.
Evidence:
1. Staff #2 fingerprints expired on 5/22/2023.
2. Staff #3 (date of employment week of February 13, 2022) . Staff #3 did not have original central registry results in record available for review before employment and caring for children.

Plan of Correction: Staff No 2 obtained appointment for fingerprints on July 25, 2023, Staff #3 new in the country applied for TIN # closely monitor the progress

Standard #: 8VAC20-800-100-A
Description: Based on children records review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #2 (date of enrollment 4/20/2021) did not have documentation of physical examination in record available for review.

Plan of Correction: All parents have been advised to provide the physical examination record on time.
Parent of child #2 has been advised to provide the document

Standard #: 8VAC20-800-120-B
Description: Based on staff records review, the provider did not obtain the required information in each staff records.
Evidence:
1. Staff #1 begin employment the week of February 13, 2022. The staff member did not have documentation of two or more references as to character and reputation as well as their competency were checked before employment.

Plan of Correction: Ahsan Khan detail to maintain each staff record up to date.
Staff has been told to bring one more reference letter about her character and reputation

Description: Based on observation and staff records review, the provider did not ensure an assistant 18 years of age and older who is alone with children in care have a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
Evidence:
1. Staff #3 was left alone with the children for the day and did not have a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

Plan of Correction: Make sure that staff working with children should have CPR and First aid certified
Staff #3 will go for training on August 5
Closely monitor the progress and maintain this record

Standard #: 8VAC20-800-150-D-2
Description: Based on observation and staff records review, the provider did not ensure an assistant 18 years of age and older who is alone with children in care have a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
Evidence:
1. Staff #3 was left alone with the children for the day and did not have a current certification in first aid.

Plan of Correction: Ensure children should not be left alone with staff have not certification in CPR and First aid
On August 5th staff #3 will attend in class

Standard #: 8VAC20-800-180-A
Description: Based on staff record, the provider did not obtain for each adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
1. Staff #2 Tuberculosis Screening expired on 5/17/2022 and there was no updated results in the record available for review.

Plan of Correction: Make sure that each household member record maintain properly and correctly.
Staff #2 obtained TB test and email you for review
