Comments:
A renewal inspection was conducted on 07/11/2023 between the hours of approximately 9:30 am and 11:50 am. There were 8 children present, ranging in ages from 21 months to 4 years, with 2 staff supervising. Children and staff were observed while playing outside in the backyard and getting ready for snack.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 2 staff records and 4 household members records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.011-F
Description: Based on observation, the current inspection notice was not posted in a conspicuous place on the licensed premises as required.
Evidence:
1) On the date of inspection, the findings of the most recent inspection of the facility (01/11/2023) were not posted. The inspection notice posted was dated 08/29/2022.

Plan of Correction: I will post now the Inspection Notice.

Standard #: 22.1-289.035-B-2
Description: Based on record review, the provider did not ensure to obtain fingerprint-based criminal history check determination letter prior to first day of employment for one caregiver.
Evidence:
1) The record for Caregiver #2 (start date 05/23/2023) had a fingerprint-based criminal history check determination letter dated 06/16/2023.

Plan of Correction: It was an emergency because my previous assistant left from one day to the next, then I had no choice but to quickly find another assistant and it took the process of the offices to send me the application so did not have on time. But now I already fix. [sic]

Standard #: 8VAC20-800-780-B
Description: Based on record review, the provider did not ensure to review, annually, with the parent the emergency contact information required in 8VAC20-800-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent acknowledgement of review of emergency information in the record for Child #2 was dated 02/04/2022.

Plan of Correction: I already fixed the parent already signed it.

Standard #: 8VAC20-800-170-A
Description: Based on record review, the provider did not ensure to obtain from each caregiver at the time of hire a current Report of Tuberculosis Screening form documenting the absence of tuberculosis in a communicable form.
Evidence:
1) The record for Caregiver #2 (start 05/23/2023) did not contain the results of a current report of tuberculosis screening.

Plan of Correction: She did on time the test. It was the result was lost in the mail and we have to get another copy from the clinic. We have already the test.

Standard #: 8VAC20-800-200-A
Description: Based on record review, caregivers did not complete required orientation training by the end of their first week of assuming job responsibilities.
Evidence:
1) On the date of inspection, the record for Caregiver #2 (start date 05/23/2023) did not contain documentation of having completed orientation training as required.

Plan of Correction: I already fix it was my mistake. She did the orientation training on 07/11/2023.
