Description:	Based on observation, diaper ointment was not labeled with the child's name. Evidence Diaper ointment, located in the diaper changing area, was not labeled with a child's name.
Plan of Correction:	I will make sure all topicals are labeled with the child's name.

Description:	Based on review of written emergency preparedness plan, (EPP), the plan is not complete. Evidence The EPP is still not complete. Only one of seven pages of the plan has been completed.
Plan of Correction:	I was working on the completed plan, I will make sure it is posted.

Description:	Based on review of emergency evacuation procedures a monthly evacuation drill was not practiced every month. Evidence An emergency evacuation drill was not practiced in the months of May and June.
Plan of Correction:	I will make sure to record the fire drill every month.
Standard #:	8VAC20-800-100-A
Description:	Based on review of two children's records, the provider did not obtain a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance for child #1. Evidence For child #2 start date 5/15/2023, the provider did not obtain a physical examination under the direction of a physical.
Plan of Correction:	I will give the physical to the parent and have it signed. I will make sure all physicals are signed before accepting them.
Standard #:	8VAC20-800-30-A
Description:	Based on observation, the provider did not ensure compliance with the terms of the current license issued by the department and with relevant federal, state or local laws and other relevant regulations. Evidence The provider is licensed for ages 1 year to 5 years. Child #1 8 months old (DOB: 10/23/2022), is registered in the family day home and in care during the inspection.
Plan of Correction:	I did not realize I couldn't accept a child under the age of 1 years old. I am changing my license to accept children starting at 3 months of age.
