Technical Assistance:
Provided the link for background checks and the TB screening form to facilitate requirements for household member. If children are playing in the backyard and under the age of 2 years a gate should be placed at the bottom of the steps to prevent access to the stairs. Once children are in care a posted menu should be placed on the parent board that follows USDA requirements to meet nutritional needs of young children.

Comments:
An announced initial inspection was completed. No children were in care. Walked through the home and backyard, reviewed emergency preparation, nutrition, diapering, trainings and equipment and materials set up for young children. The basement area is off limits to care as the steps are very steep leading into the basement. A household member who was not originally listed on the application will need completed background checks before issuance of the license as outlined in Phase II trainings and as required by the Code of Virginia. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on record review and observation, a household member does not have the required background checks upon application for licensure as a family day home.
Evidence:
1. It was verified that Household Member #1 was living in the home by looking at a utility bill with the individual's name and address printed on the bill.
2. The individual was not listed on the application as living in the home but has a bedroom set-up on the upstairs level of the house.
3. No Sworn Disclosure Statement, VA Central Registry Search or VA Criminal Record Check was completed upon application for licensure.
