An unannounced renewal inspection was conducted on July 19, 2023 from approximately 10:25 am to 11:450 am. Provider and assistant were present with 10 children, 2 of which are household members. Children were observed participating in art, outside play and story time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-60-B Description: Based on review, provider did not ensure 4 of 4 child records have documentation of required information.
Evidence: The following was found during record review:
1. The records for child #1 (start date 8/22/2022), child #2 (start date 2/27/2023), child #3 (start date 3/13/2023) and child #4 (start date 7/10/2023) did not have parent signed acknowledgment of receiving family day home's emergency preparedness plan.
2. The records for child #1 and child #3 did not have insurance plan number.
3. The record for child #4 did not have father's employment address, father's employment phone number and two emergency contacts (name/address/phone number).Plan of Correction: Not available online. Contact Inspector for more information.
