An unannounced monitoring inspection was conducted on 07/17/2023 to follow up on previous areas of noncompliance with the terms of the license. There were 12 children, ages 3 months- 5 years, totaling 29 points and one caregiver present upon the inspectors? arrival. A second caregiver arrived during the inspection. Two inspectors arrived for the inspection at 1:40 pm and departed at 2:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-800-900-B Description: Based on observations, the provider failed to ensure that children shall not be allowed to eat or drink while walking, running, playing, laying down, or riding in vehicles.
Evidence:
1. Child 5 (age 3 months) was observed laying down in a crib while drinking a bottle. Caregiver 2 confirmed formula was in the bottle.
2. Child 1 (age 20 months) was observed laying down in a crib while drinking a bottle. Caregiver 1 confirmed water was in the bottle.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-800-560-D Description: Based on observations, the provider failed to ensure infants shall be protected from older children.
Evidence:
1. Child 3 (age 4 years) was observed climbing on the side of the crib that was occupied by Child 5 (age 3 months).
2. Child 3 was observed placing a hard plastic doll, approximately 14 inches long and weighting approximately 2 pounds, on the face and chest of Child 5.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-800-570-A Description: SECOND REPEAT VIOLATION
Based on observation, record review and interviews, the provider failed to ensure that a caregiver does not exceed 16 points, determined by the point system in the standards, for each caregiver.
Evidence:
1. There were 12 children, totaling 29 points, present during the inspection. Caregiver 1 was the only caregiver present upon the inspectors? arrival to the home at approximately 1:40 pm. 2 caregivers were required.
2. Caregiver 2 was observed arriving to the home at approximately 1:50 pm. Caregiver 2 confirmed they left for their break at 12:00 pm.
3. The provider was previously found exceeding the number of points for one caregiver during inspections on 06/22/2023 and 06/30/2023.Plan of Correction: Not available online. Contact Inspector for more information.
