An unannounced renewal inspection was conducted today from 11:15 am-12:00 pm. There was 1 child directly supervised by the caregiver. The home, provider records, children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. Diapering procedures were also reviewed. There was an ample supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Evidence:
Child #1's record is missing parent work phone number, emergency contact #2 name, address and telephone number, liability insurance disclosure, policies for the administration of medication and provisions of the emergency preparedness plan.Plan of Correction: Inspector reviewed model forms on the www.childcareva.com website. I will go to library to print out forms and have his mom sign.
