Provided technical assistance on standard:
830A: the emergency evacuation procedures shall be practiced monthly. Two shelter-in-place drills are practiced in addition to the evacuation drills for a total of 14 drills annually.

Comments:
An unannounced renewal inspection was conducted from 9:30 - 10:15 am with the provider. There were five children present, ranging from 14 months to three years of age, for a total of 12 points. Children were observed playing with blocks. Two child records and two caregiver records were reviewed along with the daily attendance, emergency drill log, required posted information, and menu. The provider completed certification in Medication Administration Training (MAT) on 6/6/2023 so can administer medications to children in care. Standard #: 8VAC20-800-50-A
Description: Based on review of documentation, observation and interview, it was determined that the provider did not ensure all children were accurately included on the attendance report. Evidence: On the attendance record for 7/21, only one child was marked in attendance however 4 children were observed in care when the LI arrived.

Standard #: 8VAC20-800-60-A
Description: Based on review of two child records, it was determined that not all records contained all required information. Evidence: the record for child #2 (date of first attendance: 5/30/2023) did not contain the name, address or phone number for a second designated person to contact in case of emergency if the parent cannot be reached or the name and phone number for the child's physician.

Standard #: 8VAC20-800-530-B
Description: Based on observation, not all cribs in use had tight fitting sheets. Evidence: one crib that the provider stated was being used by an infant in care, had a loose sheet. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death.
