Technical Assistance:
Consulted on model forms and each of the required forms needed per child. Forms can be found on the VA DOE Licensing website https://www.childcare.virginia.gov/home-childcare.

'Immunization records printed off patient portals from physician's office need to have the doctor 's stamp. The VA health exam is also on the DOE Licensing website.

Comments:
An unannounced renewal study off of the Conditional license was conducted. Seven children were in care for 18 points with two caregivers. Observed free play, diapering, transition to lunch and hand washing. Reviewed records for children, emergency preparation, training hours and physical space. The provider was given additional support for paperwork requirements for children's records, reviewed information about increasing capacity and the need for a Special Use Permit from Fairfax County as well as assistant requirements and the need for an assistant if she has more than 16 points. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on observation, the children's lunches were not served in a sanitary maner.
Evidence:
Prior to lunch, the provider did not clean the lunch table.

Plan of Correction: Taken care of this problem.

Standard #: 8VAC20-800-960-E
Description: Based on provider statement and observation, the bottles were not labeled and dated with more than one infant in care.
Evidence:
Two infant children in care have bottles and they were not labeled or dated.

Plan of Correction: Bottles are labeled.

Description: Based on attendance documentation, the provider failed to keep a written plan of attendance each day.
Evidence:
No written attendance was taken on 7/18/2023 and 7/19/2023.

Plan of Correction: Attendance are up to date.

Description: Based on record review, the child's record was incomplete.
Evidence:
Child #1 and Child #2 were missing a second emergency contact.
Child #3's child record was not signed and dated and missing allergy documentation.

Plan of Correction: Reg wanted 2nd emergency contact.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface was not non-absorbent or cleaned and sanitized after each use,
Evidence:
1. The pad was covered in a soft cloth cover upon inspection.
2. The provider stated that surface was washed weekly or wiped down with flushable wipes. The wipes are not sanitizing and will not kill germs.
