Technical Assistance:
- Discussed the availability to submit the renewal application on the website at www.childcare.virginia.gov.
- Recommend keeping the emergency drill log visible to help in remembering to log each drill.

Comments:
A renewal inspection was conducted today, July 26, 2023 between 8:48am and 10:25pm. There were 3 children present with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, children and caregivers records, special care and emergencies and nutrition. There was no medication on sight at this time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Thank you for your cooperation.

Violations:
Standard #: 8VAC20-800-790
Description: Based on observation and interview, it was determined the family day home failed to post 911 or local dial number for police, fire and emergency medical responders in a visible area close to the telephone.
Evidence: 1. There was no evidence of posted emergency telephone numbers.
2. The provider confirmed the emergency numbers were not posted.

Plan of Correction: I will put the numbers back up.

Standard #: 8VAC20-800-830-C
Description: Based on record review and interview, the provider failed to maintain documentation of emergency evacuation.
Evidence:
1. The last documentation for evacuation drill was on 02/03/23.
2. The provider confirmed the documentation had not been maintained but stated the drills were practiced monthly.

Plan of Correction: I will put the drill log on the board so I remember to write the drills on it each month. I will be sure to write them down.

Standard #: 8VAC20-800-50-A
Description: Based on record review and interview, the family day home failed to keep a written record of children in attendance each day.
Evidence:
1. The last documentation of written attendance is dated 03/12/2023.
2. The provider acknowledged written daily attendance had not been done.

Plan of Correction: I will start today keeping a written attendance record. Violation corrected during the inspection.

Standard #: 8VAC20-800-90-C
Description: Based on record review, it was determined the family day home did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record for Child #2 (DOB 08/2021) did not have additional immunization documentation. Last update was 08/11/22.
2. The record for Child #3 (DOB 05/2022) did not have additional immunization documentation. The last update was 11/18/2022.
3. The record for Child #4 (DOB 02/2022) did not have additional immunization documentation. The last update was 08/19/2022.

Plan of Correction: I will inform the parents today that they must bring the updated immunization record this week.

Standard #: 8VAC20-800-430-B
Description: Based on observation, a pressure mounted gate was used as a protective barrier at the stair opening.
Evidence: A pressure mounted gate is used at the bottom of the basement stairs that leads into the daycare area.

Plan of Correction: I will replace a new gate that can be anchored to the wall.
