Technical Assistance:
Provided technical assistance on standard:
1) 790: posting emergency telephone numbers in a visible area near the telephone
2) 830D: records of emergency drills shall be maintained for one year
3) 920: the menu shall be kept on file one week
4) 980A: General written permission shall be obtained from the parent of each child for the provider to take the child off the premises of the family day home. The general written permission shall be on a form that lists regularly (reoccurring) scheduled trips (e.g., library, store, playground) and the driver, if the child is to be transported.
5) 980B: Special written permission shall be obtained from the parent of each child for the provider to take the child on special field trips (those not regularly scheduled). The written special permission shall specify destination, duration of trip, and driver if the child is to be transported.
Provided consultation on:
1) using the department's model form to document emergency drills
2) the emergency backup person should be a person who is not in the home, or regularly caring for the children

Comments:
An unannounced focused, 60-day inspection was conducted on from 9:30 to 10:15am with the provider and her assistant to determine compliance in areas that could not be observed/reviewed during the initial inspection and to follow-up on violations cited during the initial inspection. There were six children in care, ranging from four years to six years of age, supervised by two caregivers. Children were observed having free play with a variety of age-appropriate toys. Two child records and two provider records were reviewed along with the written attendance, emergency drill log, menu, required posted information, refrigerator and freezer thermometers, landline telephone, emergency supplies and additional childcare space in the lower level. The provider does not have certification in Medication Administration Training (MAT) so cannot administer medications to children in care. Violations:
Standard #: 8VAC20-800-830-A
Description: Based on review of documentation, it was determined that not all evacuation drills were practiced monthly as required. Evidence: there was no documentation of an evacuation drill for May or June of 2023.

Plan of Correction: Drills have been practiced but not documented. Going forward will start documenting emergency drills.

Standard #: 8VAC20-800-60-B
Description: Based on review of two child records, it was determined that not all records contained all required information. Evidence: the records for child #2 (date of first attendance: 6/20/2023) did not contain the name. address, and phone number for a second designated person to contact in case of emergency if the parent cannot be reached or the name and phone number for the child?s physician.

Plan of Correction: Will ask the parents for the missing information and going forward will ensure all records are complete.
