A subsidy health and safety (SHSI) inspection was completed on 7/25/2023. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Standard #: 8VAC20-790-250-F Description: Based on record review and interview, the licensee did not annually attend at least 16 hours of training, to include the department's health and safety update course.
Evidence:
Two provider / assistant records were reviewed. The record for provider #1 contained the most recent documentation of the department's health and safety update course which was dated 2/23/2022. The department's health and safety update course should have been completed again before 2/23/2023.Plan of Correction: The provider stated that she has started the department's health and safety update course and plans to finish it by the end of the week. This training will be completed annually prior to the month and date of the prior year's health and safety update course
