An unannounced renewal inspection was conducted on 7/13/23 from 9:55am to 10:40am with the provider. There were no children in care, so standards relating to the care of children could not be observed. The provider reports there are no children enrolled at this time. The provider record, four household member records, and two pet records were reviewed. The provider has current certification in CPR and First Aid. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 8/3/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 22.1-289.036-A Description: Based on review of the provider record and four household member records, the provider did not obtain the results of a national, fingerprint-based criminal history check for caregivers and household members every five years as required. Evidence: The fingerprint results on record for the provider were dated 3/12/18; the fingerprint results on record for Household Member A were dated 3/12/18; the fingerprint results on record for Household Member B were dated 3/12/18. Plan of Correction: New fingerprint results were obtained for the provider and all household members on 7/24/23.
Standard #: 8VAC20-800-210-A Description: Based on interview, the provider did not obtain a minimum of sixteen clock hours of training annually. Evidence: The provider stated that they had not completed their annual training. Plan of Correction: 16 hours of training will be completed and scanned to the licensing inspector. Moving forward, 16 hours of annual training will be completed each year.
