Technical Assistance:
Discussed age appropriate equipment and adhering to the manufacturer's guidelines regarding age ranges.

Discussed safe sleep and the use of blankets in cribs.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 11 children (26pts) with 2 staff onsite. The children were napping during this inspection. Observation of the program space to include the bathroom, crib room, changing area and nap room was made. One child remained up with the assistant providing supervision. The home was found neat and organized. One emergency medication need at this time due to a food allergy. A total of 5 children's records, Provider and 1 staff record was reviewed today. Certifications and background checks remain current at this time. The outside play area was observed and determined age appropriate for the children currently in care. Description: REPEAT
Based on child record review, written information was not provided to parents.
Evidence:
The following information was not provided in writing to parents prior to Child #3's first day of attendance:
Check in and check out procedures;
1. Policies for the administration of medications;
2. Requirement for the family day home to notify the parent when the child
becomes ill and for the parent to arrange to have the child picked up as soon as
possible if so requested by the home;
3. Requirement for the parent to inform the family day home within 24
hours or the next business day after his child or any member of the
immediate household has developed any reportable communicable disease,
as defined by the State Board of Health, except for life- threatening
diseases, which must be reported immediately;
4. Requirement for the child to be adequately immunized
5. Requirement for paid caregivers to report suspected child abuse or neglect
according to ? 63.2-1509 of the Code of Virginia;
6. Custodial parent's right to be admitted to the family day home any time the
child is in care as required by ? 22.1-289.054 of the Code of Virginia;
7. Presence of a pet or animal in the home
8. Address of the website of the department, with a note that a copy of this
chapter and additional information about the family day home may be obtained
from the website, including compliance history
9. Medication policy

Plan of Correction: A review of all child records will be completed and all incomplete forms will be filled out.

Standard #: 8VAC20-800-80-C
Description: Based on child record review, a record did not include all documentation for a proof of identity.
Evidence:
Child #1's record did not include the method of verification, the staff who viewed the verification and the date in which the verification was viewed.

Plan of Correction: All documentation for the proof of identity will be completed.

Standard #: 8VAC20-800-90-C
Description: Based on child record review, additional immunizations were not obtained for a child under the age of 2 years.
Evidence:
Child #1 is under the age of 2 years. The last documented immunizations were dated 8/22.

Plan of Correction: This child is turning 2 this week. Additional immunizations will be required from the parents.

Standard #: 8VAC20-800-180-A
Description: Based on staff record review, a report of a TB screening was not obtained every 2 years for one staff.
Evidence:
Staff #1's last documented TB screening was completed in June 2021.

Plan of Correction: An appointment is scheduled for next week. Results will be forwarded to the Licensing Inspector.

Standard #: 8VAC20-800-200-C
Description: Based on staff record review, documentation of training was not found.
Evidence:
Staff #2 was onsite providing supervision. Documentation of orientation training was not completed for this staff.

Plan of Correction: This staff received orientation. The staff will complete and sign and the provider will sign the orientation form.
