Comments:
An unannounced renewal inspection was conducted August 9, 2023 between 2:28pm and 3:45pm. There were 6 children present with one (1) caregiver supervising within the supervision guidelines. The children were observed sleeping/resting, playing with age-appropriate toys and materials, handwashing, toileting procedures, and eating snack. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, staffing and supervision, programing, evacuation drill logs, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. There was no medication was on sight. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions regarding this inspection, contact the Licensing Inspector, Sheila Nelson at sheila.nelson@ doe.virginia.gov

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review, the provider did not obtain documentation for a 5-year repeat background check.
Evidence: The record for caregiver #2 did not have documentation of an updated central registry check (expired on 1/5/2023). This is a repeat violation.

Plan of Correction: I mailed the central registry request on 1/23/2023 with the payment but I haven't received it. I have the receipt of my payment and the mailing receipt. I did not know I had to follow up. I was waiting on them to notify me. I will follow-up in the future on all background checks.

Provider emailed the agency during the inspection and they replied that the request failed at intake because the applicant (caregiver #2) did not have martial status on the request form. Provider sent the information to the agency during the inspection.

Standard #: 8VAC20-800-770
Description: Based on observation, it was determined the battery-operated weather band radio was not working properly.
Evidence:
1. The battery-operated weather band radio would not operate even when new batteries were installed.

Plan of Correction: I didn't know it wasn't working but I will buy a new one today.

Standard #: 8VAC20-800-790
Description: Based on observation, the provider failed to post 911 or local dial number for the police, fire and emergency medical responders in a visible area close to the telephone.
Evidence: The 911 or local dial number for the police, fire, and emergency medical responders was not posted near the telephone.

Plan of Correction: I will post the numbers today.
