Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:00 am through 11:30am. There were a total of five children (8points) observed with two caregivers. A total of six children's files and two staff/household member's files were reviewed. The children were engaged in the following: toy play, circle time, bathroom breaks, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-800-830-C
Description: Based on documentation review and interview, documentation was not maintained of emergency evacuation drills.
Evidence:
1. Documentation of an emergency evacuation drill for the month of July was missing from the log.
2. Caregiver #1 confirmed that the emergency evacuation for the month of July was performed, but not documented on the log sheet.

Plan of Correction: The provider will document the log sheet with the July emergency evacuation. Moving forward, documentation of the drill will be conducted immediately after the drill is conducted.

Standard #: 8VAC20-800-200-C
Description: Based on record review and interview, documentation of the orientation was not signed and dated by the provider of by the assistant.
Evidence:
1. Caregiver #2's record (Start date-May 2023) did not contain documentation of orientation.
2. Caregiver #1 confirmed that orientation for Caregiver #2 was conducted, but not documented.

Plan of Correction: The provider printed and completed the orientation form.
