Technical Assistance:

Discussed completing the backyard renovation project, which involves relocating the new playground structure to ensure fall zones for slides are appropriate before use by children.

Comments:

An unannounced renewal inspection was conducted today between 12:15pm-1:10pm. There were 12 children present with the Provider and 2 assistant s for a total of 34 points within the supervision guidelines. The children were observed drinking bottles, finishing up lunch, toileting, and laying down for a nap. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, attendance and emergency procedures were observed during this inspection. Description:	Based on a review of children's records, the provider did not obtain documentation of a physical examination for 1 out of 4 children by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #4 (doe: 10/3/2022) did not have documentation of a physical examination on file at the time of the inspection.

Description:	Based on a review of children's records, the provider did not obtain documentation obtain documentation that all children have been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence: 1. Child #4 (doe: 10/3/2022) did not have documentation of immunizations on file at the time of the inspection.
