Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:30 am through 10:45 am. There were a total of 13 children (29 points) observed with two caregivers. A total of six children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, games, bathroom breaks, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-800-830-C
Description: Based on documentation review and interview, documentation was not maintained of emergency evacuation and shelter in place drills.
Evidence:
1. At the time of the inspection, documentation of emergency evacuation and shelter in place drills was not available for review.
2. Caregiver #1 stated that documentation for the emergency evacuation and shelter in place drills was maintained, but could not be located at the time of the inspection.

Plan of Correction: The log sheet for the Emergency Evacuation and Shelter in Place drills will be found and sent to the Licensing Inspector.

Standard #: 8VAC20-800-40-A
Description: Based on observation, the provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home.
Evidence:
1. The family day home has a capacity of 12 children. At the time of the inspection, there were a total of 13 children(29pts) observed with two caregivers.

Plan of Correction: The one child that exceeded the capacity of the family day home was an emergency situation. The child had a complete record, but needed care at the time of the inspection.

Standard #: 8VAC20-800-60-B
Description: Based on record review and interview, children's records did not contain required information.
Evidence:
1. Child # 1's record (Start date-08/14/2023) did not contain documentation of immunizations.
2. Caregiver #2 confirmed that Child #1's immunization records were obtained before the first date of attendance, but the copy was returned to the parent for proper certification by a physician.

Plan of Correction: The parent will return the immunization records and the provider will document the record.

Standard #: 8VAC20-800-680-B
Description: Based on observation, caregivers did not ensure that children's hands are washed with liquid soap and warm running water.
Evidence:
1. Child #7's (2 years old) hands were not washed with soap and warm running water after diapering.

Plan of Correction: The caregivers will ensure that children's hands will be washed with soap and warm water after diapering and toileting.
