An unannounced monitoring inspection was conducted today between the hours of 9:30 am through 10:45am. There were a total of seven children (20 points) observed with two caregivers. A total of five children's files and three staff/household member's files were reviewed. The children were engaged in the following: circle time, toy play, outdoor play, and other organized activities. Areas of non-compliance are identified in the violation notice. Standard #: 22.1-289.035-A Description: Based on record review, and interview, the family day home did not update a criminal history record report every five years.
Evidence:
1. Caregiver #2's record did not contain an updated criminal history record check report every five years. Documentation of the last criminal history record check results were dated 03/28/2018.
2. Caregiver #1 stated that Caregiver #2 has an appointment to get an updated criminal history check completed.Plan of Correction: The caregiver has an appointment to get a new criminal history check. Once completed, the provider will email the results to the licensing inspector.
Standard #: 8VAC20-800-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #4's record (Start date-08/30/2022) did not contain documentation of a physical examination.Plan of Correction: The provider will ask the parents to obtain a copy of the most recent physical examination and document the record.
Standard #: 8VAC20-800-90-C Description: Based on record review, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements.
Evidence:
1. Child #3's record (Start date-09/12/2022) did not contain additional immunizations. The last immunization received was 07/27/2022.Plan of Correction: The provider will ask the parents for an updated copy of the child's immunizations and document the record.
