Comments:
An unannounced renewal inspection was conducted on 08/21/2023 from 10:20am to 12:05pm. There were 5 children ages 14 months to age 3 years old (14 points) with the Provider. The programming area, attendance, outdoor playground area, menus, Provider, Assistant and Household member records, 5 children?s records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in free play. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in MAT, CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 8/24/23. Please email me at angela.dudek@doe.virginia.gov with any questions.

Violations:
Standard #: 8VAC20-800-730-D
Description: (Repeat Violation) Based on observation and staff interview, the provider did not ensure that medication was kept in a locked place using a safe locking method that prevents access by children.
Evidence: A medication was stored unlocked on top of a table in the front hall where children enter the home and have access.

Plan of Correction: You won't see that anymore!

Standard #: 8VAC20-800-60-B
Description: (Repeat Violation) Based on review of 5 child records, the Provider did not obtain information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation.

Evidence: The record for Child #1 did not contain the action to take in an emergency situation for the physician diagnosed food allergies and parent indicated food intolerances.

Plan of Correction: Mother gave me new paper work that he doesn't have any allergies.

Standard #: 8VAC20-800-90-C
Description: Based on review of 5 child records, the Provider did not ensure they obtained documentation of additional immunizations for a child who is not exempt from the immunization requirements once every 6 months for children under the age of two years.

Evidence:
1)The record for Child #1 (age 2) did not contain immunizations once every 6 months for a child under the age of 2 years. The last immunization documentation contained in the record was dated 8/5/22 when the child was 15 months old.
2)The record for Child #5 (age 3) did not contain immunizations once every 6 months for a child under the age of 2 years. The last immunization documentation contained in the record was dated 11/22/21 when the child was 18 months old.

Plan of Correction: I have taxed you the paperwork. Please check it out that was given from the parent. I have new paperwork from 8/21/2023.
