Discussed protecting access to outdoor stairs when children are playing outside.

Two shelter-in-place drills are required by the end of the calendar year.

Provider should update the medication policy given to parents to reflect that she is no longer certified to administer medication.

Comments:
An unannounced renewal inspection was conducted today from 10:20am-11:20am. There were 5 children directly supervised by the provider. The child care area, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. The provider is currently not certified to administer medication. Children were observed playing freely and eating snack. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: REPEAT VIOLATION
Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
Child 3's record did not contain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Plan of Correction: I gave the parent the form today after the visit and they said they will take it to the Dr.

Description: Based on observation, children's records were not treated confidentially.
Evidence:
The supplemental page of the most recent licensing inspection dated 3/6/23, was posted on the bulletin board in the family day home. The page contains information about deficiencies in children's records and identifies them by name.

Evidence:
The record for Child 2 did not contain the child's health insurance policy number.

Based on record review, the provider failed to obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations prior to the child's attendance.
Evidence:
Child 3's record did not contain documentation of immunizations.

Plan of Correction: I told the parent when pick up and they emailed the correct one.

Description: Based on record review, the provider failed to obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements every six months for children under the age of two.
Evidence:
Child 1 and Child 4's records did not contain documentation that they had received updated immunizations every six months for children under the age of two years.

Plan of Correction: The parent signed the paper when he picked up his child.

Standard #: 8VAC20-800-210-A
Description: Based on record review, one caregiver did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
Caregiver 2's record contained documentation of five completed continuing training hours.
