Technical Assistance:

One of more background check was found to be missing/incomplete during today's inspection. The provider was informed that their renewal application cannot be processed until the missing background check results have been viewed by the inspection. Please be sure to forward results to inspector as soon as received for timely processing.

Comments:

An unannounced renewal inspection was conducted today between 11:40am and 12:50pm. There were 6 children present with the Provider for a total of 12 points within the supervision guidelines. The children were observed watching an education video about the letter M, playing freely with legos and magnet tiles, cleaning up and eating lunch (egg soup over rice, corn, pasta, and ham and sausage). Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Standard #:	22.1-289.036-A
Description:	Based on record review and interview, the provider failed to ensure that each caregiver and/or adult household member completed repeat background checks every five years. Evidence: 1. Caregiver #1's most recent fingerprint results are dated 5/15/208. 2. Household member #1's most recent fingerprint results are dated 5/15/2018. 3. The provider confirmed that repeat fingerprinting has not been completed when the background checks expired in 5/15/2023.
Plan of Correction:	We will make an appointment to get the fingerprint done again.
Standard #:	8VAC20-800-880
Description:	Based on observation, requirements for meals and snacks were not met. Evidence: The children were not served anything to drink during lunch.
Plan of Correction:	I serve them milk at snack time. I'll start serving milk with lunch.

Description:	Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #6 (date of enrollment: 96/2022) record does not contain a physical exam.

Description:	Repeat Violation Based on record review, children's records did not contain all required information. Evidence: Child #6's record did not contain the families medical insurance policy number.

Standard #:	8VAC20-800-180-A
Description:	Based on record review and interview, the provider did not obtain a subsequent tuberculosis (TB) screening every two years for each adult household member. Evidence: Household member #2's most recent TB screening is dated 7/2/2021. The provider stated that they completed the TB screening but did not give the results to the provider.
Plan of Correction:	They have done the screening. I need them to give me the results. I will ask for them.
Standard #:	8VAC20-800-680-B
Description:	Based on observation, caregivers did not ensure that children's hands were washed with soap and warm running water prior to eating. Evidence: The children were observed cleaning up from playing and going straight to the table to eat lunch. The provider squirt hand sanitizer in each child's hand prior to the children beginning to eat.
Plan of Correction:	The kids eat three meals here and they wash their hands for two of the meals. I will start having them wash their hands before lunch too.
