Comments:
An unannounced monitoring inspection was conducted on 8/24/23 from 8:55am to 9:45am with the provider. There was one 15-month-old child in care, for a total of four points. The child was observed watching a television show and reading a book with the provider. One child record, the provider?s record, and three household member records were reviewed. The provider has current certification in CPR and First Aid. The provider reported that she does not give medication and understands that she must obtain certification in Medication Administration Training (MAT) in order to give medication. The emergency drill log, attendance, and menu were also reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 9/5/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 22.1-289.036-A
Description: Based on interview and review of one caregiver record and three household member records, the provider did not obtain the results of a fingerprint-based national criminal history record check for all caregivers and household members every five years from the date of the last check. Evidence: The most recent fingerprints on record for the provider and Household Member A and Household Member B were dated 8/23/18.

Standard #: 8VAC20-790-180-A
Description: Based on review of one caregiver record and three household member records, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form, every two years from the date of the first screening. Evidence: The most recent TB screenings on record for the provider and Household Member C were dated 5/20/21. The most recent TB screenings on record for Household Member A and Household Member B were dated 5/23/21.

Standard #: 8VAC20-800-100-A
Description: Based on review of one child record and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: The record for Child A, with a start date of 8/8/22, did not contain documentation of a physical examination.

Plan of Correction: The parent emailed a copy of a well check to the provider immediately following the inspection. Moving forward, this will be obtained within the required timeframe.
