This monitoring inspection was initiated by licensing staff and concluded on 08/30/2023, between the hours of 10 am to 11:30 am. There were 10 children, 1 resident child present (33 points), ranging from infant to preschool-age, with 4 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (no meds on site); special care and emergencies; and nutrition. A total of 10 children records, 3 household records, and 4 staff files were reviewed.
Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the home.

Standard #: 8VAC20-800-90-A Description: Based on review and interview, 1 of 10 children files indicated that before a child may attend the Family Day Home (FDH), the Provider did not obtain documentation that the child had been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations).
Evidence -
1. On the date of inspection (08/30/2023), an immunization page for Child #5 was not available for review.
2. The start date for Child #5 was listed as 07/19/2023. Child #5 was in attendance on the date of inspection.
3. The Provider stated (during an interview conducted on the date of inspection) that she thought she had received it from the child's parents and it was included within the Health Forms on file. Documentation of a physical examination and other parts of the Health forms packet were shown to be complete. However, the immunization page on file within this packet was left blank.Plan of Correction: A current immunization page for Child #5 will be obtained and placed on file.
