Technical Assistance:
Provided consultation on standard:
1) 920: Menus shall be dated, given or accessible to parents, list any substituted food, and be kept on file for one week.
2) 940: Temperatures shall be maintained at or below 40 degrees Fahrenheit in the refrigerator and at or below 0 degrees Fahrenheit in the freezer compartment. An operable thermometer shall be available to monitor both.

Comments:
An unannounced monitoring inspection was conducted from 10:00 - 11:00am with the provider and her assistant. There were 12 children in care, ranging from 12 months to four years of age, for a total of 31 points. Older children were observed playing with play doh and, as a group, coloring a large paper taped to the floor. Younger children were playing with age-appropriate toys and listening to music. Four child records, five caregiver/household member records, the emergency drill log, daily attendance, required posted information and menu were reviewed. The provider is certified in Medication Administration Training (MAT) as of 1/18/2023 and can administer medication to children in care.

Description: Based on review of documentation, and an interview with the provider, not all shelter-in-place procedures were practiced a minimum of twice per year. Evidence: the last shelter-in-place drill documented on the emergency drill log was conducted: 7/20/2022.

Description: Based on review of documentation, and an interview, the provider failed to keep a written record of children in attendance each day. Evidence: when asked, the provider stated that she did not have a written attendance record.

Description: (REPEAT violation) Based on review of five provider/household member records, not all caregivers had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for caregiver #1 had a TB test/screening dated: 8/21/2022. The record for caregiver #2 had a TB test/screening dated: 3/15/2021.

Description: Based on observation, not all cribs in use had tight fitting sheets. Evidence: the white crib, which is being used by an infant, had loose sheets. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death.
