Comments:
A monitoring inspection was conducted on July 27, 2023 with a substitute provider and one assistant. There were 11 children present, ranging in ages from 8 months to 4 years, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 1 caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-580-C
Description: Based on staff interview and observation, the facility failed to provide daily age appropriate activities as required. Evidence: Upon inspector arrival at 11:40 a.m., the children were already laying down for rest. The children were restless and did not want to lay down. Caregiver 1 stated that the children typically rest until 3:00 p.m. Rest should not exceed 2 hours.

Plan of Correction: Rest time will not exceed 2 hours.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the provider failed to have a diaper pad that is non absorbent. Evidence: the plastic covering of the diaper pad had several tears exposing the soft padding underneath. The diaper pad is no longer nonabsorbent and cannot be sanitized properly.

Plan of Correction: The provider has a new diaper pad that can be used.

Standard #: 8VAC20-800-690-L
Description: Based on observation, the provider failed to empty, clean, and sanitize toilet chairs promptly after each use. Evidence: Upon arrival on 7/27/2023, the licensing inspector observed urine in a toilet chair. There were no children in the bathroom area at the time.

Plan of Correction: Toilet chairs will be emptied promptly and cleaned and sanitized after each use.
