Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:30 am through 10:30am. There were a total of 11 children (28 points) observed with two caregivers. A total of six children's files and --- six staff/household member's files were reviewed. The children were engaged in the following: toy play, outdoor play, and other organized activities. Violations:
Standard #: 22.1-289.036-A
Description: Based on record review, the applicant for the family day home did not update criminal record reports every five years for a household member.
Evidence:
1. Household Member #1's record contained an expired criminal background check results report. The criminal background report was dated 03/08/2018.

Plan of Correction: The household member will set an appointment to have the fingerprints taken.

Standard #: 8VAC20-800-60-B
Description: Repeat Violation
Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date-08/21/2023) did not contain the medical insurance and policy name.
2. Child #2's record (Start date-07/31/2023) did not contain the policy number of the medical insurance.

Plan of Correction: The provider will get this information from the parent s and document the records.

Standard #: 8VAC20-800-180-A
Description: Based on record review and interview, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis(TB) Screen form.
Evidence:
1. Caregiver#1's record contained an expired TB Screening form. the form was dated 04/10/2021.
2. Household Member's 1's record contained an expired TB Screening form. The form was dated 05/07/2021.
3. Caregiver #1 confirmed that Caregiver#1's and Household Member #1's TB screening was not current.

Plan of Correction: The caregiver and household member will go to the local pharmacy and have a subsequent TB test taken.
