Technical Assistance:
Discussed with the provider the requirements for posting- what is to be posted and where it needs to be posted. Further discussed the requirement for the provider to obtain proof of verification of children?s identity and age.

Comments:
An unannounced renewal inspection was conducted on 8/28/23 from 9:30am to 10:50am with the provider. There were six children in care, ranging in age from three-months to three-years-old, for a total of 17 points. Children were observed having free play, having their diaper changed, and doing an art project. Three child records, two caregiver records, and one household member record were reviewed. The provider has current certification in CPR and First Aid. The provider reported that there were no medications for children on site, but the provider has current certification in Medication Administration Training (MAT). The emergency drill log, attendance, and menu were also reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 9/11/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-800-830-A
Description: Based on review of documentation, the provider did not ensure that emergency evacuation procedures were practiced monthly. Evidence: The emergency drill log did not contain documentation of emergency evacuation procedures practiced during the months of April, May, June, and July 2023.

Plan of Correction: Keep updated file records on hard copy and on electronic copy and keep updated at all times monthly.

Standard #: 8VAC20-800-100-A
Description: Based on review of three child records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance or within 30 days after the first day of attendance. Evidence: The record for Child B, with a start date of 7/5/23, did not contain documentation of a physical.

Plan of Correction: All children have given an updated copy of physical/wellness check. They have also supplied verification of birth.

Standard #: 8VAC20-800-570-A
Description: Based on interview, observation, and review of child records, the provider did not ensure that a caregiver did not exceed 16 points by using the point system to determine if an additional caregiver was needed. Evidence: On 8/28/23 there were six children in care- Child A, 3 points; Child B, 2 points; Child C, 4 points; Child D, 2 points; Child E, 2 points; and Child F, 4 points, for a total of 17 points. There was no additional caregiver with the provider.

Plan of Correction: My assistant will be present when I am at full capacity point wise.
