Standard #: 8VAC20-800-830-A Description: Based on review of documentation, the provider did not ensure that emergency evacuation procedures were practiced monthly. Evidence: The emergency drill log did not contain documentation of emergency evacuation procedures practiced during the months of April, May, June, and July 2023. Plan of Correction: Keep updated file records on hard copy and on electronic copy and keep updated at all times monthly.
Standard #: 8VAC20-800-100-A Description: Based on review of three child records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance or within 30 days after the first day of attendance. Evidence: The record for Child B, with a start date of 7/5/23, did not contain documentation of a physical. Plan of Correction: All children have given an updated copy of physical/wellness check. They have also supplied verification of birth.
Standard #: 8VAC20-800-570-A Description: Based on interview, observation, and review of child records, the provider did not ensure that a caregiver did not exceed 16 points by using the point system to determine if an additional caregiver was needed. Evidence: On 8/28/23 there were six children in care- Child A, 3 points; Child B, 2 points; Child C, 4 points; Child D, 2 points; Child E, 2 points; and Child F, 4 points, for a total of 17 points. There was no additional caregiver with the provider. Plan of Correction: My assistant will be present when I am at full capacity point wise.
