A renewal inspection was initiated on 9/1/2023 at approximately 10:25am and concluded on 9/1/2023 at approximately 11:30am. There were a total of 10 children (35 points) observed with 5 caregivers present. The children were engaged in the following: toy play, games, movie time, snack, bathroom breaks, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 5 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-60-B Description: Based on review of children's records, and confirmed by provider, the telephone number for each custodial parent's place of employment was not contained in all children's records.
Evidence: There was no telephone number for each custodial parent's place of employment for Child #5.Plan of Correction: Provider will obtain the work phone numbers for each parent of the child.
Standard #: 8VAC20-800-90-C Description: Based on review of children's records, and confirmed by provider, the family day home did not obtain documentation of additional immunizations once every 6 months for children under the age of two years.
Evidence: There was not documentation of additional immunizations once every 6 months for Child # 5, who is under the age of 2.Plan of Correction: The provider will obtain the updated record of immunizations for the child.
